For hepatic function to be preserved after an extendedhemihepatectomy adequate venous drainageof the remaining liver is required. Most metastasesclose to the confluence of the superior hepatic veinsare considered unresectable because hepatic venousoutflow after resection would be compromised. In10–25% of people, the inferior right hepatic vein isof large calibre. Thus the superior hepatic veins maybe sacrificed and hepatic function preserved if alarge inferior right hepatic vein is present.
